I1. Who is eligible to participate in Wellness Week ?

Wellness Week is open to Port of Los Angeles (“POLA”) staff only. POLA staff is defined as employees, student workers, consultants, and contractors.

2. When does Wellness Week take place?

Wellness Week Program Period begins 9:00 AM Pacific Time (“PT”) on October 20, 2025 and ends 8:00 PM PT on October 24, 2025.

3. How do I participate in Wellness Week?

During the Program Period, participate in program Activities and earn points. Visit the Wellness Week website at POLAWellness.org for the Event Schedule and to sign up for Activities. Use a Rewards Passport to track points.

4. What is the Rewards Passport?

The Rewards Passport will be emailed to POLA staff on October 20, 2025. The Rewards Passport is the official document that participants will use to record and track points obtained during the Program Period. Participants must print the Rewards Passport and take it with them to program Activities to earn points. See Number 5 for more information on earning points and how to submit the Rewards Passport for prizes.

5. How do I earn points?

To earn points, participants are required to complete program Activities and will be required to obtain proof of participation from the Wellness Team. Points are given per each In-Person or Virtual Activity attended as described below:

• Lunch Time Fitness (Gym Days and Walking Days) = 5 points per session / max 20 points  

• Virtual Session = 3 points per session / max 45 points

• Health Fair Workshop = 3 points per session / max 12 points

• Health Fair Product/Service Booth = 5 points per visit / max 20 points

• Health Fair Information Booth = 3 points per one time visit / max 12 points

Before the Program Period ends, participants must submit their Rewards Passport either by physically at the designated locations or by emailing it to the Wellness Team.

6. What is the maximum number of points I can earn during the Program?

The maximum number of points you can earn is 109.

7. What prizes are available?

Prizes are divided into different levels. Limit one prize per POLA employee.

• Prize level 1 is available to employees, student workers, consultants, and contractors.

• Prize levels 2-3 are available to employees only. Prizes are awarded "as is," and there are no substitutions or cash redemptions. Prizes must be collected within the time specified or will be forfeited.    

8. Can I combine or transfer my points to someone else?

No, points cannot be combined, transferred, sold, or disposed of. Any attempt to do so will result in disqualification from the Program.

9. What happens if I do not turn in my Rewards Passport at the end of the Program?

Any Rewards Passport not submitted by the end of the Program Period will be voided. Points have no cash value and can only be redeemed for rewards within the Program Period.

10. How will the winners be announced?

Final Announcement of winners will be on October 29, 2025 via email.

11. What should I do if I experience technical issues or errors during participation?

If you experience technical difficulties or have any questions, please contact the Wellness Team for assistance. POLA is not responsible for system malfunctions or network failures.

12. What is the policy on prize transfers and substitutions?

Prizes are non-transferable and cannot be substituted unless at the discretion of the Wellness Team. POLA reserves the right to substitute prizes of equal or greater value if the original prize becomes unavailable.

13. Are there any privacy concerns regarding my participation?

Any personal information collected during the Program will be used solely for administering the program and will not be sold or shared with third parties. The Program complies with the California Consumer Privacy Act (CCPA).

14. Can the Program be postponed or canceled?

In the event of unforeseen circumstances (such as natural disasters or pandemics), POLA reserves the right to modify, suspend, or cancel the Program without notice.
